Q. What are the challenges an infertility specialist face in their journey?
Dr. Richa says, Many challenges come in the path of treating infertile couples.

First of all, due to lack of awareness, couples don’t turn up for evaluation and even if they turn then husbands are not willing to get tested as they still believe that it is the women who are responsible for childbearing and they don’t need any testing even though both are equally responsible for their childbearing.

Secondly, acceptability towards treatment is very low if they get tested and their reports suggest infertility they do not accept it and keep wandering from centre to centre to confirm their reports or consult some baba or other Ayurveda remedies to sort it out instead of opting for medical treatment which further complicates their problem.

Thirdly, if by chance a patient’s situation is complex and IVF is to be done then their acceptability towards IVF is low due to the stigma in society and they still keep postponing the procedure until they exhaust their reserves and they roam with their reports to various centres for other treatment options and in this, the female deplete her ovarian reserve and consequently when they switch to IVF after years it becomes a difficult task for an infertility specialist to give pregnancy thus lowering their chances of success.

Lastly, the couples who choose IVF have a habit of doctor shopping and centre shopping in need of who is providing the cheapest treatment by simply ignoring the fact that IVF is a procedure with a limited success rate and quality of treatment is one of the most important factors which will help them in their success. the quality of injections used for ovarian stimulation, media and lab conditions, doctor expertise, important evaluations of all associated medical disorders and their optimisation before transfer kind of protocols the doctor is going to use and many more factors if of proper quality they will have better chances of success apart from their own egg and sperm quality.

Q. What is your approach towards any infertility couple?
Dr. Richa says, The right approach of an infertility specialist is to get the wife evaluated by ultrasound and hormonal profile for ovarian reserve and the husband to be evaluated by semen analysis. Accordingly, we tailor the plan of treatment to meet the specific needs of a couple. If all reports are normal, then simple ovulation induction treatment is advised and in case of mild factors, IUI is advisable. If the reports and patient’s specific history denote IVF then only IVF is advised.

Dr. Richa says, Nowadays, with the help of advanced technology of IVF and ICSI, many couples can brace the joy of parenthood who are suffering from issues of PCOS, endometriosis, tubal block male infertility etc. low AMH patients can bear a child with their self-eggs by pooling of embryos and one who has depleted their reserves can opt for ovarian PRP’s for ovarian rejuvenation. The patients can identify their window of implantation with the help of ERA Testis and opt for personalised embryo transfers if their IVF is not working. They can bypass genetic disorders running in the family history or history of their child with the help of preimplantation genetic testing (PGT). PRP’s for uterine rejuvenation can be opted as well for patients facing issues with endometrium lining Not only females but males with nil sperm count can have their genetic child with the help of tesa or microtese which are surgical procedures of sperm retrieval through Testis.

Q. When we talk about IVF, there is still a stigma in society that it is a complicated and high-risk procedure. Will you please throw light on this aspect?
Dr. Richa says, Despite modernization in society, there is still a stigma towards IVF. Although infertility involves 16 per cent of the Indian population still only 1% of infertility couples are taking treatment due to the stigma or apprehension that it is a complicated process and something unnatural which is completely not the fact.

IVF is a simple and painless process that gets completed in 3 to 6 weeks depending upon whether a patient is going for a fresh or frozen cycle and babies born through this process are completely normal as a normal conception.

The only concern here is the success rate of conception which is 40 to 50 percent. So all couples opting for IVF might not be successful in the first attempt and they might need more attempts by opting more advanced techniques.
